Instructions for Donating from Your Donor Advised Fund (DAF)
Thank you for your support of a parish, school or agency of the Archdiocese of Indianapolis. This form is to understand how you would like the grant distribution applied. In order to process your contribution in a timely and efficient manner, please complete the steps in the order they are listed below.

This form is reviewed regularly and all steps and information included are up to date.
Step 1: Request DAF Grant from Custodian
Log into your DAF provider's online portal, select the option to recommend a grant. The Tax ID number to use for the Archdiocese of Indianapolis is 35-1018460.
Step 2: Submit DAF Grant Information to the Archdiocese
The fastest way to share information and provide clarity on the purpose of the fund is to use the form below Such information allows us, as the recipient, to assign a beneficiary to the gifts. Once all information is collected and funds received, we will transfer funds associated with this gift to the beneficiary and provide a gift acknowledgement for your contribution.
If we do not receive proper notification and designation, the gift will be placed into the general fund until all details are confirmed.
Two other submission options are available:
- Those who prefer to submit gift details in writing rather than using the electronic form that follows may do so, please note this may result in longer than usual processing times. Please email all the necessary details to osd@archindy.org
- Any physical documents should be submitted to Archdiocese of Indianapolis, 1400 N. Meridian Street, Suite 105, Indianapolis, IN 46202.
