Legal waiver: I the parent/guardian of the registrant, a minor, agree that the registrant and I will abide by the rules of the Cistercian Summer Programs and its counselors. Recognizing the possibility of physical injury associated with the normal activities of the Summer Programs, I hereby release, discharge and/or otherwise indemnify the Cistercian Preparatory School, its employees and associated personnel, including the registrant as a result of the registrant’s participation in the Summer Programs. I further grant permission to transport the registrant to and from activities during the camp including the optional bus service and I hereby release, discharge and/or otherwise indemnify Cistercian Preparatory School, its employees and associated personnel.

Medical Consent: I give consent for Cistercian Preparatory School through its faculty to secure any and all necessary emergency medical care for my child.

Photo Release: I hereby permit Cistercian Preparatory School to use, in whole or in part, the name, photographs, videos, quotes, and schoolwork of my son or sons enrolled in the Summer Programs for use in news releases, advertisements, newsletters, admissions material, the school’s website and any other media the school deems appropriate.
