I give the Jr. Cougar Sports Staff permission to post my child’s picture on the website/social media sites. I understand that my child's picture will not be sold and is not completely secure from being stolen off the internet. I do not hold Jr. Cougar Sports program, Gross Catholic HS, nor its volunteers or staff members responsible in the case that my child’s picture is taken from the internet. I understand that the purpose of posting pictures is to share with others the activities of our youth, not only through words, but actions as well.

Waiver of Liability and Disclaimer

I/We, the parents or guardians of the above named child hereby give my/our approval to participate in any and all league activities. We assume all risks and hazards incidental to such participation, including transportation to and from the activities. I/We hereby waive, release, absolve, indemnify and agree to hold harmless Jr Cougar Sports, Gross Catholic HS, and/or its affiliated organizations, sponsors, participants and persons for any claim arising out of an injury to my/our child, whether the result of negligence or from any other cause. I/We further acknowledge that Jr. Cougar Sports, Gross Catholic HS and/or its affiliated organizations do not provide medical insurance for players/participants. I/We will furnish a birth certificate for the above named child to league officials if requested.
