L1FT High School Youth Ministry Program is for all high school youth in grades 9th - 12th. Program nights are on Sunday Nights beginning with 5:00pm Mass and goes until 8:00pm starting September 2020. L1FT is a youth group experience committed to the reality that young people are an important and integral part of our parish community, and in fact are the Catholic Church. L1FT seeks to build disciples of the Church by fostering and nurturing the faith of our youth. L1FT will lead our youth in their faith journey, and walk with them in the light of Christ by bringing them together with their peers and leaders for fellowship, fun, faith formation and Spiritual direction.

Medical Treatment Release
As the parent or legal guardian, I hereby authorize first aide/medical treatment for my child in the event of an emergency which may endanger her/his life, cause disfigurement, physical impairment, or undue discomfort if delayed. It is understood that efforts will be made to contact the person listed on this form as soon as reasonably possible. In the event that the aforementioned requires my authorization for treatment and I cannot be reached in an emergency, I hereby give my permission to the physician selected by the activity leader to hospitalize, secure medical treatment, and/or order an injection, anesthesia or surgery for the aforementioned as deemed necessary.

I understand all reasonable safety precautions will be taken at all times by the parish and its agents during youth programing and events. I under- stand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree not to hold St. Monica, its leaders, employees, drivers, volunteers, or the Roman Catholic Diocese of Dallas liable for damages, loses, diseases, or injuries incurred by the aforementioned.
2020-2021 Social Media Permission Form

I understand that St. Monica L1FT Catholic Youth Ministry has created a Youth Ministry Facebook, Twitter and Instagram accounts, and uses Texting and email to facilitate Faith-based communication by its Youth Minister. I also understand that pictures/video may be taken during youth ministry activities, and published on these various St. Monica websites/social media sites.

Social Media photos, videos, and tagging

From time to time photos/videos may be used on social media, our website or in print for the promotion of youth ministry programs and events.  “Tagging” can be done by anyone on some media, (FB), (Instagram), etc., and only by administration from others.  We need to know what you wish to allow in regards to our YM social media administrator tagging.  Please check the boxes below to indicate which permissions you agree to.

Texting - Texting is used to “spread the word” about Youth Ministry events as well as ministering to students from time to time. All Diocese of Dallas Safe Environment protocols will be followed. By signing below, I am granting permission for communication via text messaging between my youth, (with the number I am providing below), and St. Monica Youth Director, Gerardo Olivares, and all safe environment cleared adults. I understand that my permission will remain in effect until revoked in writing to the Parish office.
Liability Release
CONSENT TO PARTICIPATE AND LIABILITY RELEASE
In consideration for allowing Youth to participate in youth activities and functions, I/we, the parent(s)/guardian(s)/conservator(s) of Youth grant permission for Youth to travel to and participate in youth events and activities. I/we assume all risks and hazards incidental to Youth's participation in the Event, including transportation to and from the Event. In consideration for allowing Youth to participate in the event listed above, and on behalf of myself/ourselves and Youth's parents, legal guardians, siblings, heirs, assigns, and personal representatives, I/we hereby release and agree to fully and unconditionally protect, indemnify, and defend the Parish, the Roman Catholic Diocese of Dallas, and their respective officers, agents, and employees, (collectively, “Indemnitees”) and hold each Indemnitee harmless from and against any and all costs, expenses, attorney’s fees, claims damages, demands, suits, judgments, losses, or liability for injuries to property, injuries to persons (including Youth) and from any other costs, expenses, attorney fees, claims, suits judgments, losses, or liabilities of any and every nature whatsoever arising in any manner, directly or indirectly, out of, in connection with, in the course of, or incidental to Youth's participation in youth events and activities, REGARDLESS OF CAUSE OR OF THE JOINT, COMPARATIVE OR CONCURRENT NEGLIGENCE OF THE INDEMNITEES. In the event any legal action is taken by either party against the other party to enforce any of the terms and conditions of this release, it is agreed that the unsuccessful party to such action shall pay to the prevailing party therein all reasonable court costs, attorneys’ fees, and expenses incurred by the prevailing party.

AUTHORIZATION OF CONSENT TO TREAT MINOR
I, am the parent/guardian or conservator of the minor, and as such do hereby authorize the Parish, its youth ministry leaders, employees, contractors and volunteers as agent(s) for the undersigned to consent to any x-ray examination, anesthetic, medical, dental, or surgical diagnosis or treatment, and hospital care which is deemed advisable by, and is to be rendered under the general or specific supervision of any physician or surgeon licensed under the laws of the jurisdiction where such diagnosis or treatment may be given, whether such diagnosis or treatment is rendered at the office of said physician, at a hospital, or at any other location. It is understood that this authorization is given in advance of any specific treatment or diagnosis, but is given to provide authority and power of treatment, or hospital care which the aforementioned physician in the exercise of best judgment may deem advisable. This authorization is given pursuant to the provisions of Chapter 32 of the Texas Family Code. This authorization shall remain effective for up to one year from the date of completion of this form, unless sooner revoked in writing delivered to said agent(s). In consideration of acceptance of this authorization, but without any time limitation and without any future right of revocation, I/we hereby release, defend and hold harmless the Parish and Roman Catholic Diocese of Dallas (Diocese), their officers, directors, agents, employees, volunteers, youth ministry leaders, and contractors from all claims, liabilities and loss in any way arising out of or in connection with or relating to such treatment and treatment decisions.

AUDIO/VISUAL RECORDING AND PHOTOGRAPHY CONSENT
On occasion, video recordings, audio recordings, photographic slides, and photographs are taken of children and youth during church and diocesan sponsored activities. These are utilized in newsletters, websites, event promotion, advertisements and other printed media. For good and valuable consideration, I hereby grant to the Parish the irrevocable and unrestricted right to make, use and/or publish any and all photographs, videos, and other images of me or my minor child or images in which I or my minor child may be included, now existing or hereafter made, in any case, with or without identifying subject for editorial, advertising, news, or any other purpose and in any manner and medium; to alter the same without restriction; and to copyright the same. I hereby release and agree to fully and unconditionally protect, indemnify, and defend the Parish, the Roman Catholic Diocese of Dallas, and their respective officers, agents, and employees, (collectively, “Indemnitees”) and hold each Indemnitee harmless from and against any and all costs, expenses, attorney’s fees, claims damages, demands, suits, judgments, losses, or liability for injuries to property, injuries to persons (including youth) and from any other costs, expenses, attorney fees, claims, suits judgments, losses, or liabilities of any and every nature whatsoever arising in any manner, directly or indirectly, out of, in connection with, in the course of, or incidental to the use or publication of any photographs, videos, or other images of my child, REGARDLESS OF CAUSE OR OF THE JOINT, COMPARATIVE, OR CONCURRENT NEGLIGENCE OF THE INDEMNITEES.

I confirm I am a legal parent/guardian/conservator and have the civil authority to arrange sacramental preparation and spiritual formation for the minor(s) named on this form.

Please read the Diocesan Activity Participation Consent and Release Form below and sign the form for your child enrolled in Faith Formation/ Youth Ministry at St. Monica Catholic Church.

By my signature below, l/we consent to my child's (Student) participation in activities sponsored by St. Monica Catholic Church (the "Parish"), including activities held on the Parish's premises and activities held at other venues. l/we understand and acknowledge that Student's participation in parish-sponsored activities may expose Student to contact with one or more persons or objects that have been infected with, and/or exposed to, diseases or viruses, including the virus commonly known as COVID-19. l/we understand that, as a result, Student's participation in parish-sponsored activities may expose Student to the risk of exposure to or infection with diseases or viruses (such as COVID-1 9). l/we understand that the potential risks associated with exposure to or infection with the diseases or viruses (such as COVID-19) are not fully known, but may include significant and serious illness, bodily injury, disfigurement, or temporary or permanent disability. l/we understand that the use of personal protective equipment may not fully protect against or mitigate the risks posed by Student's participation in parish-sponsored activities. I/we understand and agree that information regarding the COVID-19 status of Student and/or individuals that live in the same household as Student may be disclosed to others in the parish community, as the parish—in its sole discretion—deems necessary.

Nevertheless, having considered the risks, including those outlined in this Release, l/we consent to Student's participation in parish-sponsored activities. Accordingly, for good and valuable consideration, including without limitation admitting Student to the parish and allowing Student to participate in parish-sponsored activities, I/WE HEREBY ASSUME ALL OF THE RISKS OF STUDENT'S PARTICIPATION IN PARISH-SPONSORED ACTIVITIES, INCLUDING WITHOUT LIMITATION, THE RISK THAT STUDENT MAY BE EXPOSED TO OR BECOME INFECTED WITH DISEASES OR VIRUSES (SUCH AS COVID-19).

I/WE HEREBY, FOR MYSELF AND MY HEIRS, SUCCESSORS, AND ASSIGNS, AND ALL THOSE CLAIMING BY OR THROUGH ME, WAIVE, RELEASE, AND AGREE TO DEFEND, INDEMNIFY, AND HOLD HARMLESS THE PARISH AND ITS AGENTS, EMPLOYEES, OFFICERS, DIRECTORS, CONSULTANTS, RETAINED PROFESSIONALS, DONORS, AND AFFILIATES (COLLECTIVELY "INDEMNITEES") FROM AND AGAINST ANY LIABILITY, LOSS, EXPENSE, OR OTHER DAMAGE, INCLUDING PERSONAL INJURIES, PROPERTY DAMAGE, DEATH, COSTS OF COURT, AND ATTORNEYS' FEES, ARISING FROM OR IN CONNECTION WITH STUDENT'S PARTICIPATION IN PARISH-SPONSORED ACTIVITIES, INCLUDING THOSE WHICH ARISE OR ARE ALLEGED TO ARISE FROM THE NEGLIGENCE OF ANY INDEMNITEE.
