Authorization Form for use of Child/Youth Photographic Images2025/26I grant permission to Mercyhurst Preparatory School to use my child’s name and/or photographic image in production of any school publication and/or video, DVD, billboard, web, and/or other visual imaging for purposes of marketing the school.I understand that if, for whatever reason, at any point in time, I decide to revoke this agreement, and so notify the school in writing, all references to my child or his/her photographic image will no longer be used. I understand that the school is not responsible for access to internet information or downloads made by users using the web page prior to this removal of web references/photographic images. I further understand my child’s name and photographic image may continue to be used in any publication already printed or published prior to my revocation of the consent provided herein.I also grant permission for photos taken of my child, to be used in print, broadcast and digital campaigns to market and promote not only Mercyhurst Prep School but also Catholic schools in the Diocese of Erie.
