Quo Vadis…Where are you going? Quo Vadis is an opportunity for the young men and women in the Archdiocese to question where they are going and learn how to listen to God's call. Quo Vadis will help our youth know what it means to discern their vocation. It is a fun outdoor summer camp experience to learn that nothing is more important than discovering God's will for your life - and living it! Register early. Space is limited!

Waiver - Parental Consent Form & Indemnity
Dates attending Camp: July 19 - 22, 2020 Type of Event: Vocation Discernment Camp
I grant permission for my child to participate in the above named activity and I warrant that my child is in good health. In consideration of my child's participation, I agree to indemnify Dunrovin Retreat Center and Kiwamis Camp and the Archdiocese of St. Paul and Minneapolis from any claims or law suits brought against the Dunrovin Retreat Center / Kiwanis Camp / Archdiocese of St. Paul and Minneapolis by myself, my child or others that arises out of any behavior by my child at the event / activity described above. I also agree to pay any reasonable attorney's fees or expenses incurred by the Dunrovin Retreat Center / Kiwanis Camp and Archdiocese in defense of such a claim / law suit. I also hereby waive and release Dunrovin Retreat Center / Kiwanis Camp and the Archdiocese of St. Paul and Minneapolis from all claims and liability arising from any acts or omission by Dunrovin Retreat Center / Kiwanis Camp, the Archdiocese of St. Paul and Minneapolis or their agents with regard to any injuries or damages incurred by my child during the ordinary course of the event / activity. This release and waiver shall not apply to claims that may arise from intentional acts. As a parent or guardian, I agree to all of the above state considerations and conditions.

In the event of an emergency, I give permission to transport (my child) to a hospital for medical treatment. I wish to be advised prior to any further treatment by a doctor.
Are there any conditions or allergies that may affect the participant involvement in the Quo Vadis camp week?
Please note, if participants food allergies are significant, participant may be asked to provide their own meals / snacks for the duration of the Quo Vadis camp.
Is there any physician prescribed or over the counter medication which the participant needs to take during the Quo Vadis camp week?
Provide name, dosage and potential side effects of said medications
If the prescribed medication is classified a narcotic, it must be submitted at camp registration table and camp staff will administer to camper, as prescribed.
As Parent or Guardian, I agree to all of the above stated considerations and conditions.
Waiver - Photo Consent
The Archdiocese of St. Paul and Minneapolis Office of Vocations uses event photos to create promotional materials for future events. Photographs taken at this year's Quo Vadis camp will be used to create materials for the promotion of future Quo Vadis camps. This may include brochures, flyers, invitations, website and social media content.

Please note, the Archdiocese of St. Paul and Minneapolis has implemented safe environment regulations specifically addressed to how we can help protect all of God's children. The Office of Protection of Children and Youth, requests your signature of approval to use your son or daughter's image for future communications.


I authorize and consent that the Archdiocese of St. Paul and Minneapolis Office of Vocations be permitted to use and publish for general communications, advertising, commercial or publicity purposes, or for any other lawful purpose whatsoever the likeness of My Child and My Child's original work, including video, photographic portraits, pictures, or reproductions, made through any medium, including social or other electronic media, in accordance with the Acceptable Use Policy for Electronic Communications and the Social Media Policy, provided only the first name (not the family name) is identified if any name is used. I hereby release the Archdiocese of St. Paul and Minneapolis Office of Vocations with such use.

This consent regarding My Child's likeness or original work is valid for 3 years.

If I choose to rescind my authorization and consent, I agree that I will inform the Archdiocese of St. Paul and Minneapolis Office of Vocations in writing and that my rescission will not take effect until it is received by the Archdiocese of St. Paul and Minneapolis Office of Vocations. I understand however that it may not be possible to recall any work or photos that have been published prior to the receipt of my written rescission.
I have read the above Disclosures, Authorizations, and Releases, have had the opportunity to consider their terms, and understand them. I execute this document voluntarily and with knowledge of its significance.
Quo Vadis Summer Camp 2020 Code of Behavior
The following guideline content was prepared for Youth ministry by the Archdiocese, in Cooperation with the Archdiocese's Office for Protection for Children and Youth and Catholic Mutual Group.

Quo Vadis youth participants will:
  • Project an image Christian consideration, sensitivity, and respect to everyone and everything.
  • Project an image of Christian etiquette in their use of language, dress and behavior.
  • Refrain from inappropriate touching and verbal harassment.
  • Respect other persons and / or property, and keep their personal belongings of value with them at all times.
  • Refrain from actions that could result in injury and/or damage to property.
  • Adhere to stated curfew and "lights out" instructions.
  • Wear a Life Jacket when participating in canoeing, kayaking or rope swinging.
  • Be aware of noise levels in hallways and sleeping areas, especially later in the evenings.
  • Maintain the spirit of the event.
  • Report problems of any kind to a Quo Vadis Leader.
Quo Vadis youth participants will not:
  • Bring electronic devices to camp (including ipods, ipads, computers and cell phones).
  • Use vulgar language.
  • Possess weapons or explosive materials of any kind.
  • Possess tobacco, alcohol or illegal drugs.
  • Engage in or encourage any form of sexual activity or sexual harassment.
  • Engage in any bullying or physical, emotional, or psychological harassment.
  • Possess any pornography.
I hereby make a personal commitment to participate in the 2020 Quo Vadis Camp and to abide by expected standards of conduct listed above.
